Our care team meets patients where they are. The clinical team educates patients on the steps, both big and small, they can take to improve their health.

Social Work Services

Patients enrolled with Karoo can access our social work team, which addresses everyday needs that impact your health  such as co-pay assistance for medications.

 

Care Coordination & Support

Karoo Nurse Navigators assist patients who are working with multiple providers to help ensure coordination and compliance. Karoo also helps patients find co-pay assistance to pay for out of pocket costs associated with their care.

Virtual Care

Patients enrolled with Karoo can access our social work team, which addresses everyday needs that impact your health  such as co-pay assistance for medications.

What is the cost of participating in the Karoo Program?

There is no cost for participating in Karoo. This program is fully covered by insurance so you do not have to pay out of pocket for any co-pays or bills.

After a heart attack and new heart failure diagnosis, a patient returned home feeling overwhelmed and unsure how to navigate recovery. Through immediate post-discharge outreach and ongoing care coordination, our Karoo care team provided practical support and reassurance during a difficult transition.

Challenge & Support

The social worker explained how to contact this transportation provider and followed up with a text message recapping the resource details.
